Is B2B cold email legal in Italy?
No, not without prior consent when you write to a named person at a company. The same applies to generic addresses such as info@.

Promotional email needs prior consent of the "contracting party or user", which includes legal persons. An address being public (website, register, PEC index) does not replace consent.
What you have to do
- Get consent before the first email, also for company addresses
- Existing customers only: "soft spam" for similar products using the address given in a sale, if the person was informed and did not refuse
